At Leukaemia CARE one of our key objectives is to help you, whether you’re

a patient or carer, to learn to live with a diagnosis of a blood or lymphatic

cancer.


This questionnaire is for CML patients and their carers to fill in and will ask

you questions about taking medication. It should only take a few minutes, and

all your answers will remain confidential at all times. You are not required to

enter any of your personal information at any time.


By answering these very simple questions, you can help us to understand

how we can better support CML patients and carers. Taking medication

regularly and consistently is key to managing any condition, so we’re keen to

find out how the form of treatment you’re currently using affects your day-to-

day life.
